How To Apply

STEP ONE:

Fill out the following two forms:

Client Information Form
Waiver & Release Form
THERAPEUTIC RIDING AND HIPPOTHERAPY ONLY: Obtain a signed Physician Referral Form from your doctor.

STEP TWO:

Book a consultation.

Once you have a referral from your medical doctor, and have completed the forms above, please give us a phone call or fill out the form below. Valley’s Program Manager or a designated team member will perform an initial assessment and work with the other instructors to create a unique riding program suited to you or your child.

Please be aware that the assessment will not include time with our horses and its purpose is to determine the suitability of our program to the needs of the rider. As demand is high, it may not be possible to set up a therapeutic riding program immediately in all cases. Contact the office regarding lesson times and availability using the form below or calling us at 1 (604) 857 1267.
